An Example E-mail to your Reporting Senior

The below email is in the event your Commanding Officer has detached your unit and e-mail is the only method of contact. If your Commanding Officer is still at your command a request for mast is the proper process. 


Subj: REQUEST FOR ADMINISTRATIVE FITREP/EVAL CHANGE

Sir/Ma’am,

I am writing to respectfully request an administrative change in my FITREP/EVAL. 

Enclosed you will find my original FITREP signed by you on <<<DATE SIGNED>>> and a new FITREP with my proposed changes. As you can see I am only requesting <<<STATE EXACTLY WHAT YOU ARE REQUESTING TO CHANGE>>> As a committed <<<NAVAL OFFICER / CHIEF / PETTY OFFICER / SEAMAN>>> I believe these minor changes will help make me more competitive for my upcoming board.

I am requesting these changes because I believe that my performance was not adequately reflected in the enclosed report. Specifically I believe that <<<<HER IS YOUR CHANCE TO BRIEFLY DISCUSS YOUR PERFORMANCE AND WHAT IS MISSING FROM YOUR REPORT>>>

Respectfully request your decision on this matter prior to the FITREPS being submitted to BUPERS.
